Hello everyone, I’m Huong Giang, an instructor at ECE Language Center. Health is a familiar topic in IELTS, appearing frequently in both Speaking and Writing. However, don’t let that fool you into complacency! For essay prompts requiring deep social commentary, such as: “Should healthcare be free for everyone?”, if you rely solely on intuition, you may easily run out of ideas or struggle with weak arguments.
In this article, we will explore this topic from a fresh, practical, and more “economic” perspective to help you conquer a high band score.

Prompt Analysis & Brainstorming
Prompt:
Some people think that healthcare should be free for everyone, regardless of their income. To what extent do you agree or disagree?
Thesis Statement Strategy:
Instead of choosing between “Completely Free” or “Fully Paid,” I suggest a more balanced and intelligent approach:
Thesis: “While the notion of universal access is an admirable ideal rooted in human rights, I believe that entirely state-funded healthcare is unsustainable. Instead, healthcare provision should be tailored to an individual’s financial capacity to safeguard national budgets and foster a sense of individual responsibility.”
Advanced 3-Body Paragraph Structure:
To clarify this perspective, we will develop the essay using three main points:
- Body 1 (Concession): Acknowledge why free healthcare is supported (human rights, support for the poor).
- Body 2 (Counter-argument): Rebuttal – Why universal free healthcare is unfeasible (budgetary burden & dependency mindset).
- Body 3 (Solution/Nuance): Propose an optimal solution – The state should only provide free preventative measures, while specialized treatment should be paid for by those who can afford it.
Key Vocabulary for the Health Topic

IELTS Writing Task 2 Sample Essay
Below is a sample essay based on the strategy above. Pay attention to how I use transition words to ensure smooth paragraph flow.
The question of whether healthcare should be provided free of charge to all citizens, regardless of their financial status, is a subject of intense debate. While the notion of universal access is an admirable ideal rooted in human rights, I believe that entirely state-funded healthcare is unsustainable. Instead, healthcare provision should be tailored to an individual’s financial capacity to safeguard national budgets and foster a sense of individual responsibility.
Proponents of universal free healthcare rightly argue that health is a fundamental human right. From this perspective, a person’s physical well-being should never be contingent upon their bank balance. Providing free medical services would significantly alleviate the financial strain on underprivileged households, ensuring that socially disadvantaged families are not driven into poverty by exorbitant medical expenses. Furthermore, a healthy population is a more productive one, which theoretically benefits the economy in the long run.
However, it is an economic reality that providing high-end medical care to the entire populace for free is fiscally unsustainable. National budgets are not infinite, and the unrelenting pressure of an aging population and rising treatment costs can lead to strained public resources. Critically, a state-funded system often inadvertently diminishes the incentive for personal health management whereby individuals may feel less inclined to take care of their own health. If people know that the government will fully subsidize the costs for lifestyle-related illnesses, the incentive to maintain a healthy diet or exercise routine is diminished.
Addressing the fairness argument, it is essential to distinguish between treating existing illnesses and preventing them before they occur. A more sustainable model would involve the government offering free preventative measures, such as vaccinations and early-screening programs, which are far more cost-effective than expensive long-term treatments. By focusing on prevention rather than cure, the state can protect the public’s health without exhausting the national treasury. Meanwhile, those with higher incomes should contribute to their own specialized treatments, ensuring that the limited public budget is prioritized for those in genuine need.
In conclusion, while universal healthcare is a noble concept, it should not be implemented irrespective of income. By balancing state-funded preventative initiatives with income-based contributions for expensive procedures, governments can ensure a high standard of public health without compromising fiscal stability or personal accountability.
